Request could not be approved

What causes this issue?

The approved form cannot be written to the database. The form therefore
remains in the database in its old state.

System Response

The form cannot be approved (unless it has already been approved).

How to fix this error?

The problem may be caused by a temporary database error. Try saving
again.
