What causes this issue?

You tried to add a new entry, and when doing so, used a key value that
lies outside of the namespace you manage.
This can cause problems in later upgrades because the entry could be
overwritten by an import, or overwrite an existing entry.

If use of the key value entered is unavoidable, document your changes
carefully so that you can repeat them if necessary after an upgrade or

request with this data. You can then export this before an upgrade or
release upgrade and import it again afterwards.
