What causes this issue?

An error occurred while the system was attempting to insert a new entry
in the internal table for the relationships between the equipment
requirements &v1& of the general recipe stage &v2& and the secondary
resources &v3& of the master recipe operation &v4&.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.
