What causes this issue?

During upgrade, the insertion on the US2 version information into table
CVERS by RFMXPR46 failed.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

Rerun RFMXPR46 when upgrade is finished. This can occur after production
is live.
