No score determined for document and criterion

What causes this issue?

Errors have occurred during the processing of items for document &2 in
fiscal year &1. Since the header score comprises the scores of ALL
items, no header score can be determined.

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

System Response

No header score will be determined.

How to fix this error?

Check the errors that have occurred at item level (-> error log at item
level).
