What causes this issue?

Neither of the two exchange rate fields has been filled with a value
greater than zero, or both exchange rate fields have been filled.

System Response

The system issues an error message and will not allow you to continue with this transaction until the error is resolved.

How to fix this error?

Fill just one of the fields and the corresponding factors - that is,
either:
EXCH_RATE and FROM_FACTOR,TO_FACTOR in the case of a directly quoted
exchange rate, or
EXCH_RATE_V and FROM_FACTOR_V,TO_FACTOR_V in the case of an indirectly
quoted exchange rate.
